504 Plan

Purpose

The 504 plan alerts the classroom teacher(s) of the student(s) with a 504 on the Programs, Plans, and Interventions Report. Teachers can then access the 504 attachment eliminating the need to distribute paper copies. The 504 is easily accessible by all teachers with scope to the student. Classroom and/or testing accommodations are readily available to classroom teachers as well.

 

Make a statement in the Plan Summary box of the purpose of the 504.

Keep the Plan Team limited to the Case Manager, and possibly the Principal.

 

Set Up

504 plans in Mileposts are simple and can be set up in just a few minutes.

 First, find the student you want to create an intervention for.  Click on the student search link in the upper right hand corner. Enter the student name in the search field and then click the small arrow to search.

Click on the student name to enter their e-file.

  


Click on the Educational Support tab within the e-file.  Click on the Add Student Plan drop down.  Select the 504 Plan.

 

This will automatically open the plan and populate the Plan Name, set the Begin Date and add any users to notify and add to the plan team.  Click Save.

 

 

Scroll down and find the 504 Plan and click to enter the plan.

 

 

Click any link to modify the Plan Information, Plan Summary, Add Team Member, Add Note or Add Attachment.  Generally the actual 504 is written in another document by the Case Manager.  It is helpful to attach that document here by clicking on the Add Attachment link.  You can upload the file to this plan in Mileposts.

For the box marked Plan Team, it is recommended to have the Case Manager be the Plan Manager.  You could add one other person as a Team Member (like the Principal), but limit the Team to this.  NOTE:  Any person listed under the Team can make modifications to the 504 Plan.
