How to Access Form Builder


In Teacher Vitae it is possible to create or customize all of your district forms. The following steps will get you to the Form Builder tool.


  1. Login to Teacher Vitae

  2. Hover over More

  3. Click Admin

  1. Click Manage Forms

  1. Choose the form you want to edit 

  2. Click Edit
