Downloading and Publishing an Evaluation Form Template

To get started using forms, you need to download from our library of templates and publish a form so that it shows up in the Ad Hoc area (My Schedule -> +Add Evaluation) and/or Admin/Scheduling area (More -> Manage Evaluation Assignments). Silverback has published many types of evaluation, observation, and user artifact (self-authored form) templates, most of which are available to preview here



Downloading and publishing an evaluation form template

  1. Log in to Teacher Vitae as an administrator

  2. Hover over More

  3. Click on Admin

  1. Click on Manage Forms

  1. Click the Display Template button

  1. Choose the Template 

  2. Click Copy

  1. Name the Form

  2. Enable Tags

  1. Find Form in your list

  2. Verify that it has been added

  3. Click Edit

  1. Make sure the form is Published and Active

  1. Go to home page 

  2. Click My Schedule

  1. Hover over + Add Evaluation

  1. Find the Eval that you just added in the list

  1. Select

    1. Organizational Unit

    2. Evaluatee

    3. School Year

    4. Evaluation Period

    5. Package (Optional)

  2. Click Confirm

  1. To remove a form

  2. Go to the home screen  

  3. Hover over More

  4. Click Admin

  1. Click Manage Forms

  1. Find the form in the list 

  2. Click Edit

  1. Uncheck Publish

  2. Click Save
