Assigning Self-Authored Forms (IPLP, schedulable, signature forms)

To assign the Individualized Professional Learning Plan (or any other  self-authored, schedulable, signature forms) to staff:

  1. Log in to Teacher Vitae

  2. Hover over More

  3. Click Admin

  1. Click Manage Forms

  1. As with all forms you can copy a Teacher Vitae template or create your own custom form (see Create Forms).

  1. After selecting the form

  2. Click Edit

The following note and 6 steps are very important when assigning a self-authored form. 

Note: If any of the fields are greyed out and are not editable check the following:

  • Ad hoc can not be checked

  • There can not be an Evaluator Signature required:

    • Open Finalized Document and remove evaluators signature box 

  1. Click Published

  2. Click Active

  3. Click Self-Authored

  4. Add Signature Required by:

    1. Evaluatee 

    2. Reviewer

  5. Click Can Be Assigned by:

    1. Self

    2. Admin

  1. This will enable employees to launch the form for themselves, and it will allow administrators to be able to assign the form to employees they select.  Watch the following video to see how both methods work.
