Assigning Self-Authored Forms (IPLP, schedulable, signature forms)
To assign the Individualized Professional Learning Plan (or any other self-authored, schedulable, signature forms) to staff:
Log in to Teacher Vitae
Hover over More
Click Admin
Click Manage Forms
As with all forms you can copy a Teacher Vitae template or create your own custom form (see Create Forms).
After selecting the form
Click Edit
The following note and 6 steps are very important when assigning a self-authored form.
Note: If any of the fields are greyed out and are not editable check the following:
Click Published
Click Active
Click Self-Authored
Add Signature Required by:
Evaluatee
Reviewer
Click Can Be Assigned by:
Self
Admin
This will enable employees to launch the form for themselves, and it will allow administrators to be able to assign the form to employees they select. Watch the following video to see how both methods work.
