Adding a Form

Adding a Form

There are two ways to add forms. A premade template can be copied or a form can be started from scratch.

  1. Log in to Teacher Vitae

  2. Hover over “More”

  3. Click “Admin”

  1. Click on “Manage Forms” 

  1. On the “Manage Forms Page” there are several premade templates available to use or customize. To access the templates Click the box next to “Display Template”.

  1. The following screenshot shows some of the available templates. To exit the template page uncheck the box next to “Display Templates”.

  1. Choose the form that you would like to add and Click “Copy”

  1. Name the new Form or Keep the existing name.

  2. Check the Copy Tags checkbox if you would like to copy the Tags from the Template to the new Form.

  3. Click “Copy”

  1. The New Form can be found on the “Manage Forms” page

  2. The Form can be Viewed or Edited from the Action pane.
